We’re conducting a research study with frontline clinical and operational staff to understand the day-to-day challenges of managing high-risk patients, discharge decisions, and care transitions in hospitals and health systems. Who Should Apply Inpatient or outpatient RNs Care managers / care coordinators Case management or discharge planning staff ED clinicians (RN or APP) Nurse navigators Population health coordinators You should work at a hospital or health system and be directly involved in care coordination, discharge, ED decisions, or high-risk patient management. What’s Involved Short screener (2-5 minutes) 1:1 video interview (45–60 minutes) for selected participants Incentive: $125
